Help with custom product info fields

I am new to OSCommerce, so forgive me if this has already been posted but . . . I have looked all over for an answer, and cannot find one anywhere.

 

I am creating a store where each product requires that the customer enter in customization options for whoever the product will be for. For example, I will need to collect a name, birthdate, etc. but it may not necessarily be from the user who is logged in (may be a gift for someone). I need to be able to collect this information without relying on the comments section. Ideally, I would like to have some form fields and select boxes on the product page, and when the product is added to the cart, this information will be reflected just as normal attributes would be.

 

I have looked through multiple resources, and also checked the community contributions, and have had zero luck.

Any help would be hugely appreciated.
