Selling Plaques

Folks I must admit there is so much on this forum I am lost did a search on form and got two days worth of hits. Maybe someone can point me in the right direction. I have the products set up no problem, but since I have to have a form for the customer to give me what they want on the plaque, badge or award I don't know exactly how this can be accomplished. Maybe I have not found the right thread so need someone to point me to a method for linking to the store or cart after the form is completed or if it can all be done within OSC. Been doing it the old way for 20 years so would like to get this resolved.

Well that exactly what I need but it appears to be in Portuguese?
