i have install this individual item shipping module, http://addons.oscommerce.com/info/1333

 

everything work but getting this error when click checkout confirmation.

Well you can either look in the form, or try to install a later version the one you have is from 2003 the latest ones are last years 13 Feb 2007

i would not have used the oldest one there though
