Shipping and Tax not being calculated on Checkout

I can not figure out why shipping charges and tax are being displayed as zero ($0.00) on the checkout_confirmation.php page. I could see that maybe I messed up a tax zone but shipping is set to flat rate 5.00 but still shows up as $0.00.

 

Snapshot is from around Jan 10th. I have another site on the same server using snapshot from late last fall and it works fine. I have gone through both admins and find no difference with my setups.

 

Any leads greatly appreciated.
