qt pro & attribute images

Hi

Ive just installed these two contributions on my site, and having a few problems:

 

1) "add to cart" has suddenly started adding TWO items instead of one. this happens for all products. :o

 

2) attributes which are out of stock still appear as options in the product page.

 

3) the "remove" checkbox has disappeared from the shopping cart.

 

will be very thankful if you can give me any clues about these
