create_account.php Help Needed Please

I need to add fields to the create_account page that will get stored with the customer's profile.

 

I've tried searching and looking at the code myself, but I still can't figure out how/what I need to do to make this happen.

 

Any help would be greatly appreciated.

have you tried looking in the contributions? i have heard of some that do this, so you should be able to find something that meets your needs.
