Customer Password Recovery Continue Button has no link

I am new to php and Osc, this is my first plea for help.. I have identified a problem with a customers ability to recover their account password on the following page password_forgotten.php - hovering over the continue button shows no link, therefore, a customer cannot recover a forgotten password.. can anyone help me resolve this issue? a look at the code that defines the continue button has no <a href= reference, I believe the problem lies there, but I do not understand the coding well enough yet to guess up a fix... ( I also have locate a couple similar issues in the adding to/changing address book functions that need to be repaired as well)

 

Any coaching on this would be greatly appreciated!!
