Adding to contact page

Hi everyone.

 

I need to add "tick box" to the contact form on the contact page i.e. If you would like a free brochure please tick the box. and obviously when the info comes through to me i need to know whether the box has been ticked or not.

 

Does anyone know what code I need to use for this??

That may seem simple, but ti is actually quite a big modifcation and impacts a lot of files. It's not something that anyone can explain to you how to do it on a forum.
