declining a sale

I'm not sure if I am putting this in the right section, so I apologize if it's not.

 

I want to block a customer from purchasing from me again. He bought from me and then got a refund from my merchant account although he received the product. Is there any way I can suspend him from making any further purchases?

Thank you so much. I have just one question after reading the installation. It states "1) Install database changes". Does that mean I upload the blacklist.sql to the server or maybe make a sql database named "blacklist" ... I see that I have blacklist.sql here in the download but am unsure what to do with it.
