Extra Purchase Options for Each Product Listing?

I plan on selling an item which can feature multiple customization options...

 

I want an interface like the thing below the product listing at this site: http://www.frozencpu.com/products/6566/fan...THR.html?tl=g46

 

Where it says "Select Your Options"

 

I don't need nearly as many... but...

 

I'll explain my item...

 

I will be selling custom acrylic windows for computer case modding... and I plan to cutout popular game characters or cool designs out of a sheet of acrylic which will be sandwiched between up to two sheets of acrylic... as a standard, I want just the cut acrylic... but I also want the option to add a sheet of acrylic on top in any color choice they select... thats why I need a contribution like this.
