Special Payment via Company Budget Code

I have some unique criteria. I need to grab about 10 unique fields of information from a user at payment time & store them with the order. The fields are kind of wierd like department budget code, project id, project lead email, task #, and more. I was wondering if anyone knew of a module that I might be able to use and/or slightly adjust to meet my needs? I'm relatively familiar with how the modules work within osCommerce, but not sure if I want to re-invent the wheel here. I've looked through a lot of payment modules, but haven't really found anything that'll let me enter a buch of additional data instead of a credit card # etc at checkout. The only thing I've seen so far is the Purchase Order module which lets users enter 2 pices of info (purchase order number & company name) but it stores both in the comments field of the order which is not ideal. Ideally I'd like to have separate fields in the db for these extra items.
