Multiple Prices for products

Can someone tell me how can I set more than 1price / product?

 

I tried this contribution: http://addons.oscommerce.com/info/1700 but the problem is that after installing this, the products are displayed on rows.

 

I don't know why, maybe because I use a bought template or simply something is wrong with the contribution.

 

Can someone help me on this?
