Adding Drop Down Menu To Create Account Page

Already made the drop down appear in the correct place on the page, but i was just wondering how to make it display a list of options effectively, and what stages i would have to make to make it store on the database.

 

 

I hope someone can help with this, as this has to be the main function of my site to when this is complete.

 

 

I will need to add around 9-10 extra fields but under the same subject called 'Prescription' - please could someone guide me through the process to create a new section in my database with a list of values, and then link up to that dropdown in the create account page so it displays the options in the database.
