City Tax Rates error after installing mod

After installing 'City Tax Rates for California (for MS2)' posted by Gil_e_n (http://www.oscommerce.com/community/contributions,1240). I got the following warning;

 

'Warning: session_start() [function.session-start]: Cannot send session cache limiter - headers already sent (output started at /home2/xxxxx/public_html/includes/functions/general.php:20293) in /home2/xxxxx/public_html/includes/functions/sessions.php on line 102'

 

Also, I did a test buy on my osc and got the following error after clicking on "add to cart" button

Can somebody help me out on how to get this resolve. I really appreciate all your help.
