Shipping module question

Hi all,

 

I'm in need of a shipping module where customers can input their own shipping cost at checkout (this will be used for trade customers only), it also needs to have an option for handling fee.

 

I've had a look but can't see anything that'll suit my needs, is it possible anyone could give me a hand or any advice on how to create it.
