Problem with Add-A-Quickie

I have in stall the "Add A Quickie" from download

But it is not working. When I enter the Model to the "Add A Quickie" infobox, the page just refresh to the index.php no error message but the item is not added to cart.

I am using "Daily CVS Snapshot (12/24/2002), does this version (add_a_quickie.php,v 1.10 2001/12/19) work with this Snapshot.
