Need to build form for custom orders

I have a line of products that are custom built to the customer's specs. I want to create an order form that can be e-mailed to me for quote and fabrication. I would like to have pull down menus,radio buttons and text boxes to coolect the info.

I have tried building one in coffeecup form builder but cannot get it to work with osc.

Any Ideas?
