Want to Add User Enterable fields to Product when Adding to Cart

Hi,

 

I am working on setting up a new store and my requirement is for a few service type items that require information to be entered by the customer at the time of placing the order. So when the item is added to the cart, somewhere I want to add few fields where the customer should enter the information that is needed to fulfill the order. Some of these new fields will be free text fields and others if possible, I want to add validations/dropdowns for fields like Country, State/Province and Date. These new fields will be required fields.

 

I tried using Product Attributes options but they can be made to work for dropdown type fields but not free text fields.

 

I do not have much experience with web designing except for few modifications I have been able to successfully complete with the help of contributions and information on this forum and has been very helpful. Please help with this requirement.

Just want to add some more information.

 

It is not required that this information should be stored in the database but must be captured with the order along with the product/service being ordered.
