Need help with header tags

I installed Header tags and I need help.

Just explain me what I have to fill in the three different fields in the admin section (product definition).

 

If someone can give me an example, that would be very nice.

 

I'm selling sheetmusic, mostly for wind ensemble.

What would be an example with a clarinet quartet from strauss for example ?
