Parse error in PDF upload v1.5

Hello all,

 

I have been successful with all previous modules - but I'm unable to get this one to work properly >_< .

I receive a parse error message when I attempt to go to the catalog area. I believe I have followed the instructions correctly - since i did it twice and got the same error both times. Can anyone provide a fix or define my issue?
