Subcategory image folders - How do I get image in there

Tried a couple of things.

 

One of the contributions, but having great difficulty getting it to work.

And, directions are not working in the OSC user guide either.

 

Has anyone got any ideas or made something simple work?

 

I've been trying to get this going for days (lots).
