merchant problem - details are double posting -securepay

Hi guys,

 

I am not sure if this is a poblem with the securepay mod or perhaps the checkout process system,

 

We are using securepay for our merchant system and for some reason when a customer enters there details and clicks to proceed the details are doubled about thus declining the order.

 

Example of problem: (I altered customer details)

 

A order attempt failed!
Transaction Response:

AVS Response: NO DATA
Authorization Response: NOT APPROVED

Every order is doing this and just wandering which file would be double inputing this info?
