How can I enable CCV2 code work when checking out with authorize.net?

My checkout page did not show the CCV code table, how can I add it and enable it work? The instruction from http://www.oscommerce.com/community/contri...h,authorize.net is quite mess.

Can anyone provide a clear one? :rolleyes:
