Shipping Calulations

Is there a way to have a flat rate shipping to say $100.00USD and percent rates above that?

 

Also, how can I modify "Zones" to delivery type (ie Ground, Ground to PO Box, and DHL) and have a different rate for each?

 

Any help will be greatly appreciated.
