Customer Testimonials v2 Error 500 on submit

Hi Guys

 

I have installed the Testimonial 2 module and all works well until the user clicks submit testimonial. The next page does not display and the error: The website cannot display the page HTTP error 500 appears. Funny thing is that the tetimonial is actually written and sent to the admin control panel.

(This is the testimonial page, the problem occurs when the user clicks the send button)

 

Can anyone suggest why the page is not displaying and how I might be able to fix it?
