Address Not Showing at Checkout (Resolution)

I was having a problem with the delivery address not showing on checkout. Doing a search about the problem, I noticed several times the problem was posted but no real answers came from it. (Most are now locked in the General forum.) So, I thought I would post here what solved the problem for me.

 

Actually it ended up to be pretty dumb. After searching through the coding for anything I might have changed, I finally looked at the database itself and found that I had somehow emptied the address_format table. Just restoring the data to that table resolved the problem!

 

Of course this might not be the same resolution the others needed, but obviously it should be something that is checked.
