Need help with Zone shipping

A few months ago, I installed a zonesworld contribution to allow multiple zones for shipping. I know I tested it out and thought it was working fine. However, recently a couple of real customer orders came through, and instead of a shipping calculation, this text was posted in the order: "Zone Rates (The shipping rate cannot be determined at this time): $0.00"

 

I've looked at my Zone rates, and I cannot see anything wrong there. Both orders were for US customers (we are a US company) and US is my Zone 1 country. Here's how my Zone Rate settings look:

 

Offer zone rate shipping: True

Tax class: none

Sort Order: 0

Zone 1 Countries: US

Zone 1 Shipping table: 1:5.00,2:8.00,4:10.00,5:16.50,6:18.00

Zone 1 handling fee: 0

 

Then the other zones are set up for other countries (CA, MX as Zone 2, and a larger listing of them in Zone 3).

 

Any ideas why this is not working?

I think I just figured it out. Both orders happened to be for multiple quantities of the same product. That product had its weight entered very high (8.0 instead of .50), and my shipping table doesn't go up to 8 (pounds). I'm doing some testing on it, but I'm 90% sure that was it.
