Comment Field text fo an Item?

Hello, i'm asking if there is some modules that allow the customers to add a text comment for every product that he choose.

 

For example, I buy an host for 40% and i have an option for a domanin registration for 15 U$, ok, i'd like that the customer specify the name of the domain that he choose. Is there something already coded?

 

If there is not, tell me what files I have to code and what database table and i'll code for the community. I'm pretty new here and not a lot experienced in this tool, for now ...
