Attributes not being recorded on some orders

Recently a few orders have started coming through with sizes or colours missing as if the attributes were not recorded.

 

eg orikaso comes in red, blue, olive etc but when you view the order it just says "orikaso" where it should say "orikaso - colour:red"

 

Its only on a few orders and never the same product.

I havn't managed to do it myself.

 

I've done a search in the forum & found a very simular problem, however they were using EP & I'm not.

 

any thoughts?
