i have a question regarding this contribution: i managed to install it and it works fine, but i want to add 2 more new fields and i'm not sure how to modify this

INSERT INTO configuration VALUES ( '', 'Display Product UPC', 'PRODUCT_LIST_UPC', '1', 'Do you want to display the product UPC?', '8', '11', '', '', '', '');

to match those 2 fields i want to add. anyone could give me a hint?
