Has Anyone Used This Contribution And....

I was wondering if anyone has used the following contribution:

First Item $X, Additional Items $Y Shipping

And has set this up for multiple types of products. Such as:

 

Product Type A: $5 for the first item plus $2 for each additional item.

Product Type B: $7 for the first item plus $4 for each additional item.

Product Type C: $10 for the first item plus $6 for each additional item.

 

If so, I would really appreciate it, if someone would let me in on how you do it. Thanks =)
