How to add a pulldown menu to shipping

I have searched high and low and I am unable to find what I am looking for.

 

I want to add two required pulldown menus to checkout_shipping.php then display the information on chekcout_payment.php.

 

This mod is going to be used to specify a time for a catering order to be delivered and a time for the equipment to be picked up.

 

I am currently using Shipdate_2_3 for choosing the date.

 

Any help would be very much appreciated.

Since nobody has answered this one,

how about some help on how to make the comments field required.

I'll just use that for the delivery and pickup time for now.
