Shipping Address not Showing in Shopping Cart

When I try to checkout the shipping address is not showing in my shopping cart. Here is a screen shot:

There are addresses in the customer address book but they do not display

I actually did some more searching today and found a solution (in case anyone else has the same issue):

The address_format table was empty and this post has the SQL to add to the DB
