Fill-in fields for products

I need to set up at least three fill-in fields for each product (Team Name, Player Name, Jersey Number), and I need the information input to carry over to the shopping cart as part of the description. Any ideas?

there is a contribution for this called Imprint Text Options here http://addons.oscommerce.com/info/2618

 

and another one here http://addons.oscommerce.com/info/160 which looks better than the above but again wont play nicely with QT Pro

 

 

I too am looking to do the same thing but i have QT Pro 4.3 + fast_easy_cross_sell_V1.1 installed and it seems that Imprint Text Options wont play nicely with the ones i have installed already.

 

So im still looking for a solution.
