Need table rate, but per item, with 2 levels

My client wants to ship her items like this:

Large - 1st for $6, each additional for $1

Small - 1st for $3, each additional for $1

 

I've been at this all day so it might be staring me in the face, but in any case, I need help! Thank you so much if you can help out! :)

I've not used it, I was just looking thru the contrib's and saw it.

I installed it and it's so close, but I can't seem to modify it to have the 2nd tier shipping. It'd be awesome if I could modify it so it went by a product code instead of zone, but I'm clueless!

I've tried everything I can think of for this, does anyone have any ideas?
