can't create new user accounts

The user fills out the form, hits done, and gets the confirmation screen. From there, the page only refreshes when done is selected, and no account is created. There are no error messages. The db is installed and seems to work on all other functions. Any ideas?

I have the same problem but can't solve it I have version 2.1.
