But when clicking the button "Confirm Order", the total is changed to $264.25 and the Flat Rate cost is missing. Sub-Total: $264.00 Flat Rate (Best Way): $3.00 Sales Tax for California: $0.25 Total: $267.25 Double check your Order Totals module through your admin panel and ensure that there are no order totals with duplicate sort orders.
