HELP need phone numbers in shipping address

I am in dire need of some assistance ......Please help me

 

I need two phone numbers added to the shipping address form.

One for a daytime phone number one for a night time phone number.

 

Please any help with this would be greatly appreciated.

 

I'm not very experienced with php and mysql but can follow directions well :)
