Need to make "fill in" field on product attribute

For example, an item can be personalized with 5 lines with a limit of characters or spaces in each line. How do I do this?

 

Other questions:

 

Can multiple selections be set up? I.e. If the customer wanted to pick 2 colors- instead of making 2 attributes, can I have just one and allow multiple selection?

 

Also, can radio buttons be created instead of drop down menu?
