I need a Customer Feedback form

I need a way to get feedback from customers. I have been working on the site http://www.skinsgameclothing.com and its basically complete, the only thing missing is a customer feedback form. The other sites my company owns use Yahoo! shopping cart which they are trying to get away from due to its skimming cash off the top on each order.

 

But one feature Yahoo! shopping cart does have is a customer feedback form. During the check out process, the customer is provided the option to recieve a feedback form to let us know how were doing. I basically want to do the same thing... I found one contrib but i don't think its quite what i need.

 

If you have a way of getting feedback thats been working well for yourself, let me know! thanks a BILLION!!!
