Missing graphic on Category's page

Hi,

On the right hand side of the header is a broken box where a graphic should go. I don't see anything in the index.php file where I can designate the graphic's name. When I look at the source code for the page in Firefox, I can see plainly in the html where I need to add the file name, but don't know which file is generating that part of the page.
