Flat shipping on first item, discount on subsequent?

Would it be possible to do something like this? If so, can anyone tell me how?

 

example: Per order, the first item purchased is $7.00 for shipping, every item after that is $1.50.

you know the total products amount, lets say 7.

 

You do 7-1= 6 times discounted fee + 1 time expensive fee = total fee.
