There is something along these lines in the contributions section - hack it about to suit your needs. Look for "clean up images"

Yes 2.2-CVS does :) I recently added checking into the tep_remove_product() function to cater for products that share images. If the image is used by another product than the one you are removing then the image does not get removed.
