Help! $5 being added to UPS Shipping costs

When using the UPS shipping module, an extra $5 is added to the cost of shipping for all shipping classes (residential ground, overnight, etc). The only place I can think of that it's coming from is the Flat Rate shipping.

 

I tried settting Flat Rate to $0.00 as well as removing it in the admin tool, but no luck. $5 is still being added.

 

I confirmed this by submitting the string sent in ups.php and comparing the results in the raw return vs. the return in the checkout_shipping.php on the screen. I'm using the snapshot from 11/10/02.

 

Any suggestions? :?
