Search Forms?

I have found a number of search form contributions (Category Drop Down lists, etc), but all of the submissions that I have seen all populate a big clunky nav bar box with the form. I'm trying to integrate the search boxes into my own top nav bar (HERE).

 

Using straight HTML (as in the View Source) loses the userID and isn't as dynamic as I'd like it to be.

 

Does anyone have any down-and-dirty, naked php that will simply produce a dropdown list of categories without any formatting?

 

(Same request for keyword search)
