Hi everybody,

 

I want to add one custom form in osc on shopping cart. Basically process is.... when your add the product in the cart and press "Check Out" button it will check customer logged in or not. after sign in it will show you the "Delivery Information" page. But i want to show here my custom form where take some inputs and based on these inputs order cost changes.

Here you have the "Proceed with order". when you click this button you will see this form. this is the form i want to place between "Check Out" and "Delivery Information" pages and also impect on order price....
