problem with adding an address

When a registered customer goes to the address book and clicks on "add address", a blank page comes up, no errors or anything and it's all blank (no boxes or anything).

This happens with all customers.

I run v. 2.2 build sometime march 2002 and don't use SSL.

The store is running fine, and after all this time I was notified of this problem by a customer.
