How Can I Add "where Have You Heard About Us?" To Signup Page?

Hello,

 

I want to start flyer advertisements in several german cities and would like to know how the ads are working after that.

 

Thus it would be great if someone knew how to add a text box to the signup screen, where the new customer must tell me where he has heard about my shop.

"Please tell us how you got notice about this shop", or something like that... this information should be viewable on the customer.php page or somewhere else where I can look up where my customers come from. ;)

 

English is not my native language... so I don't know how you call this kind of thing.
