To change date format and include mob phone also while registering

1) I want to change the date format as dd/mmm/yyyy-25/jun/2007

 

2) want to include mob no. option while any new user is registering, where mob no. is optional but as attributes as telophone no.

 

3) In contact us page want to include a pop up at the right side having:

office/store: Address

Factory location: Address
