Confirm order

I do have a few pay modules installed. When testing, when clicking the confirm button, it proceeds to next next just fine.

 

But if I icheck cod or check/money order and the the confirm button, I get an error - "webpage cannot be found - http error 404"

 

 

any ideas any one?
